My Holistic Care

Home & Community Support

Home & Community Support helps individuals live safely and independently in their own homes while remaining actively connected to their communities. This service recognises that wellbeing is supported not only by physical assistance, but also by routine, social connection, and a sense of purpose. Support is tailored to individual preferences and focuses on enabling independence rather than replacing it. Individuals are encouraged to participate in tasks where possible to maintain confidence and daily living skills.

Our trained support workers provide assistance respectfully and at a pace that suits the individual. Community participation is an important part of this service, supporting individuals to maintain relationships with family, whānau, and friends, and engage in meaningful activities. This service is suitable for older adults, people living alone, individuals with disabilities, and those requiring short-term or ongoing assistance. It does not include personal hygiene or medical care and is delivered within agreed support plans and scope.

Support may include:

  • Light household tasks such as cleaning and laundry
  • Meal preparation support and grocery shopping
  • Assistance with errands and appointments
  • Support to attend social or community activities
  • Encouragement to maintain independence and routine
  • Social connection and community engagement
